Members not currently enrolled in the plan should complete the UCC Medicare Advantage Plan Enrollment Form and return it to PBUCC with a copy of their Medicare card via email to This email address is being protected from spambots. How to Register for MyHumana

Step 1:

• Click the Medicare link
• Enter your Humana Member ID number
• Type your date of birth
• Type your five-digit ZIP code
• Read the Online Services Agreement and Humana Web Confidentiality Agreement and click Continue to accept the terms

Step 2:

• Type and confirm your email address
• Create your username
• Type and confirm a password
• Select a security question and response
• Click Submit. The procedure is complete.
